Iliopsoas impingement: a newly identified cause of labral pathology in the hip

Purpose

To describe iliopsoas impingement as a newly recognized cause of labral tears located anteriorly near the iliopsoas tendon, occurring without bony abnormalities.

Methods

A retrospective review of 25 patients who underwent isolated iliopsoas tendon release and labral repair or debridement, with at least 1 year follow-up. Clinical and intraoperative data and outcomes were analyzed.

Key Findings

  • Labral tears adjacent to the iliopsoas tendon occur due to compression or traction by the tendon.
  • Surgical release of the iliopsoas tendon with labral treatment improved hip function and pain.
  • Outcome scores showed good to excellent results at about 21 months follow-up.

Conclusion

Iliopsoas impingement is a distinct pathological entity causing anterior labral tears, effectively treated with tendon release and labral repair.

If you have anterior hip pain and labral tears without bone deformities, surgery to release the iliopsoas tendon may relieve your pain and restore hip function.
