Arthroscopic Iliopsoas Fractional Lengthening for Internal Snapping of the Hip: Clinical Outcomes With a Minimum 2-Year Follow-up

Purpose
To evaluate outcomes of arthroscopic iliopsoas tendon fractional lengthening for patients with internal snapping hip undergoing hip arthroscopy.
Methods
Prospective case series of 55 patients undergoing arthroscopic IP fractional lengthening with minimum 2-year follow-up. Outcomes measured by patient-reported scores (NAHS, HOS-ADL, HOS-SSS, mHHS), pain (VAS), and patient satisfaction.
Key Findings
- Significant improvement in all patient-reported outcomes (P < .001).
- 82% of patients reported resolution of painful snapping and good/excellent satisfaction.
- Patients with snapping resolution had significantly better outcome improvements than those with persistent snapping.
Conclusion
Arthroscopic IP fractional lengthening is effective for treating painful internal snapping of the hip, though a minority may experience persistent symptoms.
What This Means for Patients
This procedure can relieve painful hip snapping and improve function for most patients undergoing hip arthroscopy for labral or impingement issues.
